Clinical document translation underpins claims decisions that must withstand audit
An insurer assessing a claim arising abroad decides on the basis of documents produced in another language and within another health system. The hospital report, the discharge note, the diagnostic results and the clinical invoice are the evidence on which the reimbursement decision rests — and that decision must still hold up months later, when it is reviewed in audit. At GT-Traduções we have worked with clinical documentation for four decades, from our office in Loulé.
International mobility has made this commonplace. Foreign residents treated in Portugal, Portuguese nationals treated abroad, travel policies, private health cover and occupational policies with international scope all generate a steady flow of clinical documentation that must be read precisely and filed intact.
Does your company assess claims involving foreign documentation?
We work on a continuing basis with fixed terminology, agreed turnaround times and a confidentiality circuit defined from the outset.
Contact our teamThe documentation most often found in claims files
- Medical reports and discharge notes — these describe the clinical episode and establish the basis for cover.
- Diagnostic and test results — they support the diagnosis relied on in the file.
- Invoices and itemised clinical statements — each procedure must be identifiable and match the report.
- Certificates of temporary or permanent incapacity — these determine how compensation is calculated.
- Prescriptions and treatment plans — they justify continuing or rehabilitation costs.
- Emergency and transport documentation — admission reports, transfers and repatriation.
- Expert assessments and second clinical opinions — where the file calls for independent validation.
What an insurer requires of a translation
Traceable figures
Amounts, dates, procedure codes and units are checked one by one. A single mistranscribed figure compromises both the reimbursement calculation and the later audit.
Documentary integrity
The translation reproduces the document in full, including stamps, signatures and handwritten annotations identified as such. Nothing is omitted or summarised.
Stable terminology
The same diagnosis is named in the same way across every document in the file, so the assessment never has to hesitate between synonyms.
Need predictable turnaround for recurring volumes?
We set turnaround times by document type and a secure submission route, so that claims assessment never waits on the availability of a translation.

Frequently asked questions
Does claims documentation require certified translation?
It depends on the use. For internal assessment, standard translation is normally sufficient. For files subject to audit, litigation or submission to official bodies, certified translation is commonly required. We fix that criterion with you at the outset.
How do you keep documents in the same file consistent?
We maintain a glossary per company and per file, so that the same diagnosis and the same clinical procedure are always named in the same way across every document.
How is policyholder information handled?
Within a restricted circuit, accessible only to the translators assigned to the work and under GDPR. We sign confidentiality agreements whenever the company requires them.
Can you handle recurring volumes to fixed deadlines?
Yes. For clients with a regular flow, we agree turnaround times by document type and a submission route in advance, so the service keeps pace with assessment.
Do you also translate non-clinical documents in the file?
Yes. Invoices, correspondence, declarations and administrative documentation attached to the claim are handled within the same file, using the same terminology.
